Unless the real reason for his struggles is that I have him on two of my fantasy teams. Needless to say, no matter how bad he gets, I'm keeping him.

either his age caught up to him, or he is being distracted, or he has lost his focus and combinational pitching scenarios towards hitters, it happens, in his mind he might think i need more pitches to disguise his main pitch.
I think that over the years the batters have figured him out. They just wait until he falls behind and they sit on a pitch over the plate. But then again on the last two HR he gave up (Scutaro/Beltre)...he was ahead 0-2 & just made bad pitches. Boston has just killed him lately...could it be because of all the games they play against each other since they changed the scheduling a couple of years ago?
I think he needs to develope some type of breaking pitch that he can sneak in once and a while. That would make his cutter and fastball look faster. Batters would have to guard against a breaking pitch and would not be able to sit on just a fastball.
